Register to Learn How to Better Manage Your Diabetes!
Would you like to learn more about managing your diabetes, monitoring your blood glucose, and using medications safely?
Diabetes cannot be cured but it can be managed. The solutions are not easy and usually take effort and accountability. We would like to join your journey for diabetes control through self management using the Diabetes Conversational Map. The goal in using the Conversational Map is to provide you with tools to become a better self-manager of your diabetes disease. In groups of 3-10, you will go through an interactive learning workshop series that helps you to effectively understand diabetes related information and draw new conclusions.
The five U.S. Diabetes Conversation Maps will support you in understanding many of the basic facts associated with diabetes, the relationship between diabetes and healthy eating, the value of monitoring and using the results, the natural course of diabetes, and ways to self-manage your own disease.
The class is sponsored by the Allegany County Wellness & Prevention Committee, Allegany County Office for the Aging and Allegany/Western Steuben Rural Health Network. Classes will be held at the new Allegany County Office for the Aging facility at 6085 Route 19 North, Belmont, NY 14813. Facilitating the class will be Registered Dietician, Cheryl Czworka, and Registered Nurse, Chris Mountain. The class is free and pre-registration is required by contacting Melissa Williams at the Allegany/Western Steuben Rural Health Network, Inc. at 585-593-5223 ext. 15 by Monday, July 26, 2010. Each class will build upon the previous week making it extremely important to attend all sessions. Classes are scheduled for Tuesdays and Thursdays throughout August; including the 3rd, 5th, 10th, 12th and 19th.
